I/we authorize our suppliers and banks to release all pertinent credit information.

If credit is granted, I/we understand that the terms of the sale are net thirty(30) days following purchase, unless otherwise agreed. KE WA PA will charge interest on any past due balance at the rate of 1.5% per month or 18% per year.

In consideration of KE WA PA extending credit to the above business, I/we do hereby guarantee, jointly and individually, to pay all rental charges and charges for other goods and services supplied to any of us or to the above business. I/we agree to pay all costs including reasonable attorney's fees, court costs and finance charges incurred by KE WA PA in the collection of this account.

All payments received on this account will be applied first to cost of collection, second to finance charges and third to the oldest invoice.

In the case the above company is a corporation, I/we personally guarantee to pay any charges incurred by the corporation.
